A well-designed pre-appointment questionnaire will be presented. The information gathered from this questionnaire can be used to create a personalized dental environment tailored to the patient's needs. This includes preparing the operatory by accommodating any sensory perception disorders, understanding the patient's communication abilities as well as addressing the expectations and concerns of the parent/caregiver.

Be able to identify significant information necessary to accommodate patients with special health care needs and disabilities.
Be able to prepare a dental operatory to accommodate those with sensory perception disorders.
Be able to set realistic goals so the provider can tailor the visit without overwhelming the patient.
