436 - Sorenson, C. University of Nevada Las Vegas, Las Vegas Nevada Conservative Restorations For Endodontically Treated Permanent Molars In Pediatric Patients

Abstract:

Introduction: Full coverage restorations, such as stainless steel crowns, are the gold standard for root canal–treated molars in children. However, when insufficient tooth structure remains, a more conservative interim approach may be necessary. A glass ionomer buildup reinforced with an orthodontic band can provide an effective temporary solution.

Case Report: A 15-year-old female presented to the UNLV Pediatric Dental Clinic with pain and sensitivity in the lower right molar. Tooth #30 was fractured to the buccal gumline with extensive decay and visible pulp. Vitality testing confirmed Symptomatic Irreversible Pulpitis and Symptomatic Apical Periodontitis. Root canal therapy was completed, followed by a Fuji II RMGI buildup. Due to limited remaining structure, the occlusal was reduced by 1.5 mm, and a size 36½ orthodontic band was cemented.

This conservative approach preserved tooth structure and provided stability for future porcelain crown placement.
