389 - Retrospective analysis of efficacy of extractions in early childhood caries

Abstract:


Purpose:   To assess the efficacy of second primary molar extraction, versus restoration or pre-fabricated crown, in patients with ECC as a means to reduce overall caries on permanent first molars over the following one year.



Methods: This retrospective cohort study included patients aged 3–10 years who received treatment for caries on primary second molars (direct composite restoration, prefabricated crown, or extraction) between 2010 and 2024. Bitewing radiographs within three years of initial treatment were required. Radiographs were assessed for caries on adjacent permanent first molars only if no restorative treatment had been performed there during follow-up. Patients who had restorative treatment in the three-year period did not have their bitewing radiographs reviewed by the study team. Descriptive statistics were calculated, including chi-square and Fisher’s exact test, and a generalized linear mixed model was used to evaluate the effect of extraction versus composite restoration or stainless-steel crown on carious lesion incidence on adjacent permanent first molar; with statistical significance set at P < .05.
