441 - Perspectives on Dental Care Transition for Adolescents with Special Needs

Purpose: Adolescents with special healthcare needs (ASHCN) face unique challenges when transitioning from pediatric to adult care. This study explored legal guardians’ perspectives on preparedness, confidence, and perceived barriers.



Methods: A mixed-methods design was used. Nineteen guardians of adolescents with special healthcare needs (ASHCN) completed surveys and semi-structured interviews. Data were analyzed descriptively and thematically to identify key transition-related experiences and concerns.

Results: Satisfaction with pediatric dental care was high (median = 5.0). Preparedness for transition and confidence in adult care were neutral (median = 3.0). Completing treatment before aging out was strongly endorsed (median = 5.0). Confidence in transition resources was moderate (median = 3.5). Qualitative analysis identified three themes: strong attachment to pediatric providers, perceived lack of training and accommodations in adult care, and structural barriers to transition.

Conclusions: The transition from pediatric to adult dental care for ASHCN is influenced by relational, clinical, and systemic barriers. In this sample, limited transition planning and adult provider availability may delay transition. The development of coordinated, patient-centered transition protocols, enhanced provider training, and expanded pediatric dental capacity may improve continuity of care and long-term oral health outcomes.
