Abstract: Erythema multiforme (EM) is an intriguing clinical entity characterized by an acute, self-limiting mucocutaneous hypersensitivity reaction. It may arise due to various etiological factors such as infections, medications, autoimmune disorders, and malignancies, with herpes simplex virus (HSV) being the most common infectious trigger, accounting for nearly 90% of cases. This report describes a case of EM in an eight-year-old female child who presented with an acute onset of crusted ulcers involving the lips and oral cavity, accompanied by sparse cutaneous lesions around the perioral region. A detailed clinical history revealed prodromal symptoms followed by rapid disease progression. Careful clinical examination of the lesions, supported by serological testing, confirmed the diagnosis of herpes simplex virus–associated erythema multiforme (HAEM). The patient was managed with appropriate medications and supportive care. In conclusion, thorough clinical evaluation combined with serological confirmation of HSV-1 association enabled accurate diagnosis and resulted in uneventful management.
